Pilot error led to the crash of a Louisiana National Guard Black Hawk helicopter in March, which killed 11 U.S. service members. The Irish Times newspaper cited in June 3 issues an official investigation into the crash, which occurred off the coast of the Florida panhandle while the aircraft was conducting a nighttime training exercise. The dead included four Louisiana Guard members who served as the aircraft’s crew, and six Marines. One of the Marines, Sgt. Liam Flynn, was an Irish national. The report determined that the pilot of the stricken helicopter and another aircraft disobeyed an order and flew in low clouds where visibility was poor.
