The Pentagon acknowledged that ordnance from an air raid intended to target hostile elements in Kunduz, Afghanistan, instead hit a hospital operated by Doctors Without Borders, killing medical staff and patients. Defense Secretary Ash Carter promised a “full investigation into the tragic incident,” in an Oct. 3 statement. Army Brig. Gen. Richard C. Kim will head the investigation team. During an Oct. 5 press briefing, U.S. Forces Afghanistan commander Army Gen. John F. Campbell said that the attack was launched at the request of Afghan forces. He has ordered a complete review of rules of engagement.
