
A third of IG recommendations from a sampling of past reports that the EPA lists as resolved in fact are still open, an audit found.
In a sampling of recommendations from seven prior reports, the IG said that was the case for 15 of the 48 in which the EPA had certified it took corrective actions.
These included recommendations to better track funding in “brownfields” programs; screening programs for chemicals that can disrupt human hormone function; risk management and incident response; and response to contaminants in Flint, Mich., water.
“Inaccurate data in the audit tracking system limit the OIG’s assurance that it can rely on the status of corrective actions reported by the agency” and affect the accuracy of the office’s reports to Congress, the report said.
EPA management replied that the report may have been missing information and data that may have been relevant and suggested that the IG’s office may have reached different conclusions if it had sought clarification from the agency.
The IG disagreed, noting that the report consolidated findings from several prior follow-up reports, and that the EPA had concurred in new recommendations that reiterated those previously made.
