Certain individuals who may believe they are eligible for payments under the settlement were not mailed the packages, however. These might include survivors of deceased class members who were not designated for survivor annuities; they must submit a “Potential Successor-in-Interest Form” to the Settlement Administrator. To get that form, mail a letter with your name, address, and Social Security number to the Settlement Administrator, P.O. Box 4068, Portland, OR 97208-4068. Others who believe they are in the eligible class but who do not get a distribution package should request a “Class Membership Claim Form” by sending name, address, and Social Security number to the same address. Those requests, too, must be postmarked by October 27.
