Where to send SF2809 Out of Season

I’m a retiree who needs to change his retiree health coverage from Self +1 to Self only. I have an acceptable QLE. Where do I mail / email / fax this form out of season? I can’t use the online system. Thanks.

First, if you are retired, you need to use OPM Form 2809, https://www.opm.gov/forms/pdf_fill/opm2809.pdf, and mail it to Office of Personnel Management, Federal Employees Retirement System, P.O. Box 45, Boyers, PA 16017-0045
