DFAS mailed 1099Rs by Dec. 29 to retired members and Survivor Benefit Plan annuitants, but numerous telephone calls and e-mail messages DFAS has received indicate that many did not arrive. DFAS offers several solutions. For example, trace down the envelope containing the annual Retiree and Annuitant Statement because the 1099Rs were mailed in those envelopes. In addition, myPay users can go to www.dfas.mil and use their Personal Identification Number (PIN) to access myPay and print the 2004 1099R. Individuals with computer access who do not have a PIN or who have forgotten it, can use the same site to request a PIN. Those with computer access also can go to Custom Inquiry Request by visiting www.dfas.mil. The nature of the inquiry would be “Missing 1099R.” If all else fails, contact a DFAS customer service reprehensive toll free at (800) 321-1080 and request the 1099R be reissued.
