Four soldiers died Nov. 23, when their UH-60L Black Hawk helicopter crashed at Fort Hood, Texas, during a routine training mission. The four were identified as: Sgt. 1st Class Toby A. Childers, 40, of Hays, Kan.; Chief Warrant Officer 3 Stephen B. Cooley, 40, of Cantonment, Fla.; Sgt. 1st Class Jason M. Smith, 35, of Destrehan, La.; and Chief Warrant Officer 2 Michael F. Tharp, 40, of Katy, Texas. They were attached to 2nd Battalion, 291st Aviation Regiment, 1st Army Division West. The helicopter was attached to 7th Battalion, 158th Aviation Regiment. A team from the Fort Rucker, Ala.-based Army Combat Readiness Center is investigating the cause of the crash.
